Hi Dr S.,
I emailed you a week ago with the info you requested. I am hesitant to post my scans etc on a public forum so I sent them to your private email address.
I do appreciate you looking at these because I know you are jolly busy! However pls be aware that further treatment for me is being put on hold pending your response.
I am deteriorating quite significantly and quickly so your soonest response would be much appreciated.
If you are unable to respond please let me know - I do understand and I remain grateful for your offer of assistance.

Just a suggestion... under the circumstances you describe, I would call his office if I were you.

We haven't seen him here for a few days now, he has got to be swamped between ISNVD last week and the SIR conference where he is hosting a workshop next week.
