Go to your survey and you'll find that in the drop down.

There is also a monthly option at $19.99 and you can upgrade at any time.
Have you used surveymonkey before?

Sarabellum - that is close, but still focused on group aggregate. I was trying to find a way to use sur. monk. as a short cut to a score (average) on each individual reply. I will look for some script code that might do this.

Of course folks can do this just fine by printing the MSIS or FSS and scoring it themselves, but I've always found it is easier if you can just ask folks to click through a survey and get a score at the end.

Marie, I think this is a GREAT idea!

Thanks to everyone who is implementing this!

I'm going to keep track of this on my calendar, too...that way I can, also, tell if there are events that seem to consistently make me feel worse than others...I think Dake, as well as, our neuros and maybe even our GPs might be interested...

The problem with the MSIS is that it needs to total all 5 columns, then add the sums together to get the score, then show the score to the person taking the survey (these instructions will be added to the Tracking template, until I come up with an automated score survey).

I found a perl script that does exactly this. Now I just have to figure out how to install this on my webserver. Any Unix folks want to give me some tips, like how to set chmod 777 on a directory?

I have sent you a PM.

Configuring a web server to permit execution requires more configuration than chmodding to 0777. Anyway, chmod -R should apply the mode to a whole directory.
