where are the lesions located?

My question may seem stupid but where exactly are the MS lesions? when we say white matter, are we talking about cranial nerves?
I keep looking at MRIs, pics on the internet but still don't understand...
Are those lesions literally brain lesions or lesions of the nerves? They sure are separate entities... For example we can have lesions on the spine, that does not mean we have lesions on the nerve coming out of that place...
We can also have nerve inflammation without lesion, for e.g optic neuritis, that mens an inflammed optic nerve not necessarily a lesion on that nerve ...
and nerves are supposed to be part of the peripheral nervous system (while brain is central...). Can anyone explain all this to me?...
