PAID Survey - H.P. Acthar Gel Patients

Hello,
I am conducting a short survey for patients who have taken H.P. Acthar Gel. The survey is part of a project focusing on specialty drugs, being conducted on behalf of one of our clients. Those completing the brief survey will be compensated $50. We would require participants to sign a short consulting agreement prior to payment. This agreement states we are not asking you to reveal any proprietary or nonpublic information.
To be clear, all information from this survey will not be published, sold or otherwise shared beyond this client. Moreover, as a regulated entity, we adhere to strict laws and regulations.
