Need help finding a LDN link

Somewhere in my research I found a site where someone had put together an information packet about LDN you could print out to show your MD. It looked like there was a lot of good info there.
I can't seem to locate it again. Anyone know what I'm talking about or where to find it?

Have you seen the files prepared by Maureen? She carefully researched LDN and selected pertinent info to present to her orthodox neuro whom she managed to convince to write out an LDN prescription. These files are available at

In addition, there is an 'extra' piece to be added to the doc's file which is also very impressive and scientific- looking. Here it is (but it takes time to download)

Maureen has given her consent to reproduce these files in the hope that we may convince a few more doc's to prescribe LDN instead of us turning up empty handed and being rejected.
