Dr. Poblete speaks about the TOS / CCSVI relationship

Our remarkable findings

Some moths ago, while I was taking a look to the original Zamboni images [...] I realized that they were similar to those we had (for TOS) some years ago [...]

When I realize this, (because those images with stenosis with unknown cause from Zamboni) that all of them were in the lower part of the jugulars and precisely in the thoracic outlet region, I thought that those stenosis were produced by the presence of a undiagnosized severe TOS

Then I started considering the possibility of surgical decompresion trying to remove the reflux, in the normal way for severe TOS, i.e. cutting a portion of the scalen muscle and drying the first portion of the first rib, according to the Roos procedure.

In the several TOS with MS cases we have treated by now (16) the evolution has been the same that in those TOS without MS. A fast and progressive healing of the many symptoms, of which the TOS seems responsible, instead of the MS itself.

Meses atrás mientras observaba las imágenes originales del Dr. Zamboni [...] caí en la cuenta que eran enteramente similares a las que obteníamos nosotros años atrás [...]

Al advertir a través de ésas imágenes que las diversas estenosis descritas por el Dr. Zamboni como de causa no precisada, y que a su entender explicaban el reflujo venoso estaban localizadas todas ellas en la parte más baja de las yugulares y precisamente en la región del opérculo torácico, terminé por convencerme que éstas estenosis eran provocadas únicamente por la presencia en todos los enfermos con EM de un SOT severo no diagnosticado [...]

A partir de éste análisis comencé a considerar la posibilidad de descomprimirlos quirúrgicamente intentando eliminar sus reflujos yugulares, pero haciéndolo de la manera como lo hacemos habitualmente en los diversos SOT sin EM que terminan finalmente en cirugía, esto es seccionando en cada caso el escaleno anterior y resecando el arco anterior de la primera costilla por vía axilar, tal como lo recomendaba Roos.

En los diferentes SOT con EM que hemos operado hasta ahora y que numéricamente son pocos ya que recién alcanzan a dieciséis, y todos también muy recientes y con poco seguimiento puesto que los primeros recién llevan cuatro meses, la evolución inmediata no ha diferido absolutamente en nada de la que hemos observado en forma constante a lo largo de los años en los SOT sin EM operados, en todo lo que dice relación con una rápida y progresiva desaparición de los diversos síntomas que presentan y de los que parece ser responsable únicamente el SOT más que la EM misma.

I just talked about TOS/CTOS on my Dr. Mehta thread that lists lots of the symptoms.... Vascular surgeons, such as Dr. Mehta (it's a sub-specialty for him) can perform the open operations necessary for this condition (IRs cannot).

I bet we're going to see more and more of TOS/CTOS in the literature, just like we did with May-Thurner Syndrome.

Have you got any feedback from Dr. Mehta? Does he think that the CTOS connection is possible?
