Interesting, patientx, thanks
Analysis of cervical plethysmography data in diagnosis of chronic
cerebro-spinal veinous insufficiency
PROFESSOR SIMON J. SHEPHERD, PHD
FELLOW OF THE ROYAL SOCIETY OF MEDICINE
The presentation describes the initial mathematical and physical analysis of patient and
control data from the new cervical plethysmography diagnostic technique, introduced
by Professor Zamboni, in the diagnosis of CCSVI.
We show that the data produced is consistent with previous recent work which showed
that the hydrodynamics of the brain is very closely approximated by a second-order
system.
We show that a classical two-parameter characterisation of the second-order response
curves to the Heaviside step function could be a powerful classification technique for
diagnosis of CCSVI.

Dr. Shepherd's abstract on plethysmography from ISNVD.

And the graph from Dr. Zamboni's work that Cheer linked above, complete with an awfully cute human figure to mark the switches between supine and upright:

this chart --- ^ ---- looks very similar to Mr. Success ..... BEFORE ... cocktail hour ...... COCKTAIL HOUR ....... then after cocktail hour ... but the giveaway is the color of the heart ........ Mr. Success' heart is thought to be black .......

Kudos to Dr. Simka! He has a trial underway, comparing diagnostic value of doppler sonography, MRV, and plethysmography.

great news. They will study another 200 people. I believe this includes healthy controls ?

It starts this summer [ August ] .

Dr.Simka will include the use of the plethysmography neck collar.

I am confident in Dr.Simka. He has been a brave CCSVI pioneer medical professional.

Let's give him our support.

I also would like to tell him and his team: thanks, great, congratulations and I wish good luck to the trial!

Does anyone have more info on this ???
My ideal senario is for the neck collar test before the CCSVI procedure showing reduced flow then after flow is normal. It would be a cheap non invasive method to check patents as follow up, as well.

Primary Outcome Measures:
•Positive and negative predictive value of Doppler sonography, MR venography and air-plethysmography [ Time Frame: All non-invasive test are done up to 14 days prior to catheter venography ] [ Designated as safety issue: No ]

Positive and negative predictive value of Doppler sonography,MR venography and air-plethysmography for the detecting of vascular malformation in the internal jugular veins with catheter venography as the reference test
It seems fairly straight-forward. All noninvasive tests, which would be the plethysmography, MRV, and doppler, are done first. Then catheter venography is then done as the reference test.

He is looking at diagnostics beforehand, not what might be improved after treatment. Treatment is not mentioned in the study at all, although I would assume it is done.

I applaud Dr Simka for running this trial, and the help he was here on TIMS and to others. I do have a little niggle regarding the fact he is not using the "Gold Standard" of IVUS.

I wonder if Dr. Simka has access to IVUS? We've seen two abstracts now that show more stenoses were found using IVUS: Dr. Sclafani's abstract that will be presented at SIR and the German abstract presented at a conference a few months ago.

There was also another one, not specifically CCSVI, by a Dr Singh (from US) I think, who found considerably more cases of May-Thurner in symptomatic patients using IVUS over standard venography.

Dr. Raju also presented on the use of ivus in may thurner at ISNVD, and I think it was 90% stenoses seen on IVUS vs 50% seen on regular flouroscopy. It's in the isnvd 2012 abstracts. Really interesting, especially since it's being debated whether IRs should check these veins. I'm glad mine were checked.

But I came here to post the following from the ISNVD press release:
A New Device for Rapid Non-Invasive Assessment of Cerebral Venous Return was Presented: An ideal tool to screen for CCSVI among patients affected by neurodegenerative disorders, known as cervical plethysmography was announced. It is also useful and cheaper to monitor the result of endovascular treatment of CCSVI after the operation.
An ideal tool. Is this available in hospitals? Or in clinics? If Dr. Cumming has it at HCMC, I would happily volunteer to get checked out.

Well, it took over a year to get published (!) But here it is-- Dr. Zamboni's blinded study, showing the flow diffrences in normals vs. those with CCSVI--
