A lot of these people may have longstanding progressive multiple sclerosis or they may be very stable patients with benign or burnt out multiple sclerosis. Still others may prefer to avoid drugs based on personal preference. The 25% of patients who are not aware of DMTs being available to them may be older patients with long-standing progressive multiple sclerosis with no MRI activity. These patients are not very good candidates for DMTs anyway. There is only one approved drug (Ocrevus) demonstrating unambiguous benefit, and the benefit was only 24% vs. placebo. Also, in the ORATORIO trial, the average age was 44, and many patients had superimposed relapses or gad+ lesions, so a 60 year old without active lesions is much less likely to benefit. The drug also carries significant risk.
This article is funded by a drug company trying to argue that the NHS is under-treating people. This may be true, but the claim is a gross exaggeration. Someone could write another article about hundreds of millions of dollars wasted on the inappropriate use of older DMTs in 70 year olds with progressive MS and EDSS >/= 7.

One reason is because of studies showing that the DMTs really don't offer much in the way of long term benefit in the treatment of MS. But be very wary of reports sponsored by these drug companies.....they are in business to make as much money as possible and that is their bottom line.
