Natalizumab as a first line of treatment any advice?

Hi All,

I had been recently diagnosed as a case of Multiple sclerosis with active inflammation and had my first attack a month ago. Intravenous steroids were given and am currently on Oral steroids for tapering dosage. I had paraesthesias and visual symptoms in right eye, but after steroid therapy, currently i dont have any symptoms except for Urgency and frequency of urination, no incontinence though. My Neurologist after reviewing my clinical profile and MRI progression of lesions opined that I should be started on Natalizumab as a first line treatment for 2 years followed by switch over to other medications. However I couldn't decide whether to start with the third line of drugs straight away nor could find any inputs from the people who were started likewise.

Can anyone who is also on similar treatment with Natalizumab as a first line treatment as opposed to other first line drugs share their experience?

Can anyone please tell me if you dont mind what were your presenting symptoms and the rationale your neurologist discussed to start on Tysabri straight away instead of other treatments?

And another major concern to me is if required in future if I need to be put back on Tysabri after the switch over, wont it actually increase my risk of PML? and was it discussed with you whether starting straight away on Tysabri, exhaust options for next line of managements in the long term future or not?

Are you JC virus antibody positive?
If so, do you know your JC virus antibody index?
If you are JC virus antibody negative, what would be the reason for switching you to another drug if you are stable?


tysabri as a first line treatment is not uncommon.

My JC antibody status isnt done yet. But my Neurologist is of opinion that irrespective of that result I should go on Tysabri for atleast an year or two.

Hi,

I was also prescribed Tysabri as a first line after my initial diagnosis. I have been on it now for a few months. (Monday will be my 7th infusion). All is well.

I am JC negative, so it made the choice easier for me.

When I asked my MS specialist why she recommended Tysabri for my first DMD, she said due to the "significant" amount of spinal lesion involvement I have. She didn't say this, but reading between the lines, my impression was that she was concerned that even one relapse could mean impairment for me.

Hello, new here. I was diagnosed in Nov. My neuro also chose tysabri as my first med. Fortunately i am JC neg, due to the severity of my diagnosing relapse and the locations of my lesions we both decided it was the right choice. I've only had one infusion so far, due for second one next week. I have been slowly recovering. Can't be sure if it's the med right now. I have done tons of research and have read promising things. Hope everything works out for you!
