• Adults with biotinidase deficiency often exhibit myelopathy, paresis with or without optic neuropathy/vision loss.
• Individuals thought to have multiple sclerosis often exhibit similar clinical findings.
• Biotinidase deficiency is readily treatable with biotin.
• Therefore, biotinidase deficiency should be considered in individuals thought to have multiple sclerosis or related disorder.
