• No biomarker is available for predicting treatment response and neuroinflammation in Multiple Sclerosis.
• IFI35 increases from the untreated to IFNbeta-1a treated MS patients, compared to HCs.
• IFI35 evidences predictive characteristics respect to lesion burden, WMF and formulation type of IFN.
• IFI35 discriminates the responder to IFN therapy from the non-responder patient.
