But maybe your MS is not as bad as it would have been without all that time in the sun? There's always wiggle room like that.I mean, I was a sunbather all of my life and my Mom never used sunscreens on me or anything and I still have MS.

Presumably, given Dr. Compston's research, the twin with the MS would have had some immune-system related genetic abnormalities and the twin without MS, being identical, would have had the same, therefore the twin without MS would be atypical if they were a randomly chosen healthy control, but they were not randomly chosen.
It does clearly say that genetics does not determine everything. So when Dr. Compston says, "We now know what causes MS," and he is pointing to the genetic differences, that is not enough to be the cause. Sensitivity and specificity, Dr. Compston.... ;)
