My name is Helen and I am doing an Extended Project Qualification at college based on treatments for Multiple Sclerosis. Welcome to ThisIsMS. Regarding your survey, there are several questions that need to be reworked. For example, question 5 needs to be reworked. I suggest having 3 fields in one line for each med and the ability to add more lines as needed. These fields could be a drop down box for the year, a text box for the med's name and a check box for either prescribed or recommended. Questions 7 and 8 need to be reworked as well. It's best to ask only one question in each survey question so split these two up. Multiple fields could be used here as well.
