Highlights

• This article brings original data from a real-world practice and compares the diagnostic performance of MRZ reaction, OCB, oKFLC and KFLC index in MS.

• Compared with OCB, oKFLC and KFLC index, the MRZ reaction demonstrated the highest diagnostic specificity, but had the lowest sensitivity.
• On the contrary, OCB showed the high sensitivity and specificity, and thus it remains the golden standard CSF biomarker for the diagnosis of MS.
