Conclusion
In conclusion, we utilized high-resolution echo-less 3D-MRSI at 7T to investigate the topography of metabolic changes in a large brain volume of MS patients. Our findings revealed distinct patterns of metabolic alterations outside lesional pathology, with widespread NAA/tCr reduction, especially in motor, sensory, and cognitive regions, and elevated mI/tCr levels concentrated around the lateral ventricles.
Metabolic ratios involving NAA correlated with disability scores, thereby highlighting the impact of neuronal damage on disease progression. Further large-scale longitudinal studies are needed to validate the potential of 3D-MRSI for enhanced monitoring of disease activity, progression, and treatment efficacy.
