The 4th edition is out, dated 2006. It's nearly 1000 pages of fine print and pictures. Scary and informative. Here is a list of the chapters:

The Story of Multiple Sclerosis
The Distribution of MS
THE Genetics of MS
The natural history of MS
The origins of MS: a synthesis
The symptoms and signs of MS
The diagnosis of MS
The differential diagnosis of MS
MS in the individual and in groups: A conspectus
The neurobiology of MS
The Immunology of infammatory demyelinating disease
the pathology of MS
The pathophysiology of MS
The pathogenesis of MS: a pandect
Care of the person with MS
Treatment of the acute relapse
The treatment of symptoms in MS and the role of rehabilitation
Disease modifying treatments in MS
The person with MS a prospectus

I only just got it, it's clearly a reference and not a cover to cover read. While I don't think it's a necessary book to have, I think it's a good supplement, especially if you are a "I want to know more" type of person.
