I have been on Rebif for about a year. I realize relapse can be pretty random but chances are one is going to happen relatively soon. If I feel I am having a relapse, should i inject the medication during the exacerbation?

Most definitely. Its the interferon that helps reduce the frequency, duration and severity of relapses.

I would also increase intakes of omega 3, vit D , resveratrol, and quercetin all of which compliment the interferon. I have done this each time I thought a relapse was coming on and seemed to head it off.
