I was looking at vilnietis doc on his levels and noticed his creainine went up from 72 before treatment to 86 mmol/l but 17 months, later came back down to 78.
Also, research has indicated the D is renal protective.

Mother was on Rituximab and prednisone for Microscopic Polyangiitis which attacked the kidneys and lungs.
Drs denied her dialysis and appropriate treatment for her MPA and forced her into hospice.

Well, it depends on each person.
But generally, it does not change. At least I never saw people complaining about their creatinine levels.
As you already know my brother takes huge doses (250,000 iu's) and some of his creatinine levels were:
10/21/14: 1 mg/dl
24/10/14: 0.9 mg/dl
08/10/15: 1.2 mg/dl
08/19/15: 1.1 mg/dl
02/27/16: 1 mg/dl
09/06/16: 1.10 mg/dl
Reference values:
Men.: 0.66 to 1.25 mg/dl
Women: 0.52 to 1.04 mg/dl
I do not have their 2017 exams on my laptop. But I know that it did not change because he would tell me.

It does look like some fluctuation happens.
Do you know if there is a maximum calcium intake allowed by the protocol?
I know that calcium will make the PTH come down and could cause a false calculation on the D requirements.

The fluctuations seem very small.
When he was taking 180,000iu's (08/10/15 - 1.2 mg/dl) creatinine was higher than when he started to take 250,000iu's (02/27/16: 1 mg/dl)
First, the patients need to reduce their calcium intake and follow a low-calcium diet.
Then after 2 or 3 months (for patients without kidneys problem) doctors check patient's exams and do an adjustment if necessary.
It works analogously as the vitamin D dose adjustments.
