I rarely sleep on my back - always preferring odd positions on my face or side, all twisted up. I've noticed in the past that this is detrimental to sinus pressure/congestion, and recently noticed how much external pressure the pillow puts on my neck in many of my preferred positions.
Anyone think there is any chance this could contribute to jugular occlusion/reflux?

I had been thinking the same thing since when I slept on my side, my stent that you can feel subcutaneously feels swollen in the mornings. I started using my Temper-pedic contour pillow and have been sleeping on my back. It helps because once your head conforms to the pillow, you don't move and you stay on your back.
I thought the same thing and just decided to give it a go. Now this is just me "measuring" how much that area is swollen simply by touch and saying "that feels smaller this morning" so take that with a grain of salt, but I have taken to sleeping on my back now and I really have to say it has helped with morning congestion also.
