The reference indicates this was the IJV for a hemodialysis patient for whom previous cannulations may have been through this IJV. This could leave a bit of scarring which, combined, with the tendency for the flow through curves and stenoses to be helical in nature (google "helical flow index"), may ultimately cause a twisted vein like this (like the banks of a river are reshaped by meandering flow). This is my SWAG (scientifically wildly applied guess).
