If anyone reading this is, or knows someone who is, capable of translating a two-page document into French, it would be much appreciated if they could tell me by PM. Thanks.

Have you tried Google's language translator? It's often not perfect, but should get you close if you can copy and paste the text into their form. Once you get a rough translation, someone fluent in French should be able to easily smooth out any rough spots.
