Can you please help me with my research project?

My name is Lisa and in early 2011 I was diagnosed with RR MS. Since my diagnosis I have tried to stay positive and keep myself busy. I have gone back to school to gain my Masters degree in Human Resource Management, for my dissertation topic I have chosen to research the dilemma of workplace disclosure of individuals who have been diagnosed with this disease.

The questionnaire will take about 5 minutes to complete and I would really appreciate any help with this topic.
