Welcome to TMI, formerly TIMS yuk yuk yuk. Takes a good four dayer to get through just the highlights here! Happy perusing. Don't neglect the stickies upstairs they are quite a valuable resource.
Many of us talk insurance stuff on our individual threads, some threads are just insurance. There's a ton of stuff on here already, just a matter of pulling out the info in a timely manner.
Of course there is a big diff between "needing referral" type insurance and self-referred type PPO's like what I had, so I'm no expert by any stretch, but first it would be incumbent to get your GP on board and in the loop about what you would like to do and why, attacking it from the "fixing bad veins" angle rather than "treating MS". (ONLY, not one or the other but a mixture of both!)
If-it-aint-broke-don't-fix-it does NOT apply here...
Here's a few threads I dug up:

Can you tell us what HMO you have?
Everyone as far as I know that has had the stenting has had coverage for the stenting regardless of company. It's a matter (of course) of how much, in network out of network in state out of state and all that...
Welcome!
