When I read the announcement about the Canadian MS Society funding a study on pogressiveMS one of the comments was that they wanted to look at the damage caused prior to inflammation. Instead of thinking that the inflammation causes the damage the inflammation may be a RESULT of damage. So the disease process and injury starts before the inflammation is seen on the MRI. this is their hypothesis if I understand correctly.

I have the same question. My MRI images have not changed in the last three years, but my symptoms are worsening in severity. I am secondary progressive, first symptoms in 1981, diagnosed with MS in '87.LR1234 wrote:What are the MRI's not picking up??

Gray matter damage? Atrophy? I can't think what else. Wallerian degeneration of the nerves. Iron accumulation, as questor suggested.
White matter lesions are not a perfect way to measure this disease, but they are an accepted way.

just to remind everyone once again ...... on their webpage ... the NMSS explains that
on autopsy ..... . brain lesions were /are found in people that lived normal lives until
death.
The great invention - The MRI - while a wonderful tool ..... can yield false information.
To allow neurologists to use brain lesions as a score card for MS ....... isn't right.
The better Neurologist's know very well .... what show's up on MRI's ..... could be one
of many different problems.
